FLU IMMUNISATION FORMS
The forms went out at the beginning of the week to all pupils. Please would you ensure that if you haven’t already returned your child’s form, that it is with us by Wednesday 12th September at the very latest.
Even if you don’t want your child to have this immunisation we still require the completed form back with your choice selected. If your child does not return the form then they will not receive the immunisation this year.
